Default Soyuz Failure

Comrades:

For those of you wondering, the Soyuz failure a few days ago appears to be due to one of the strap-on boosters (Russians refer to them as 1st stages) colliding with the sustainer (2nd stage) immediately after separation, approximately +118 sec. after launch. Either the pyrotechics at the base of the stage failed or an oxidizer valve, which should have provided retrograde thrust as the booster fell away, failed to open and the tip of the booster sliced into the sustainer causing the failure.

The capsule separated from the booster using solid rockets on the still-attached fairing as the escape tower had been jettisoned just a few seconds prior to the failure. The descent module separated from the fairing and the habitation module and made a ballistic descent. Both the astronaut and the cosmonaut are fine.
